Santa Clara County Homelessness Prevention System
Santa Clara County Homelessness Prevention System (HPS)
HPS is a unique public-private partnership, led by the County of Santa Clara's Office of Supportive Housing in partnership with the City of San Jose, Destination: Home, and Sacred Heart, and implemented by a network of 19 community partners. The system identifies households at risk of experiencing homelessness and provides a range of financial assistance, legal support and case management services to help them remain housed. HPS partners work together to develop, implement, and scale a countywide prevention model and system of care that is focused on outcomes and best practices in preventing further displacement from the communities we love.
